Entry forms are available in the premiums, however if you prefer a blank AKC entry, please print the above form and mail to: Margaret Bleakney 612 North Ross Avenue Mexia, Texas 76667	Fax both the entry and agreement to: (254) 237-2158 If you fax your entry, your check/money order or PayPal funds must be received before your entry deadline.	Use the PayPal button below to send Entry Fee listed from desired show premium to margaretbleakney@nctv.com. $2.50 processing fee required per entry. Then you may FAX or email a PDF signed entry form including the back to margaretbleakney@nctv.com. Please note in the PayPal notes your dog's AKC # to help speed up the process.
